POSITION: 6352B-Dietician (Full Time Regular)

LOCATION:  Health Center for Women – Hawthorne, CA

DUTIES: The Dietitian will provide comprehensive nutritional care to patients identified with special dietary needs, and/or who meet nutritional risk criteria. The Dietitian also conducts patient assessments, education sessions, and plans of care. • Completes Dietary assessment, writes on Progress Notes in the patient’s E.H.R., information such as counseling, food substitutions and diet instructions provided to the patient or any pertinent impressions of the patient’s ability to cope and/or acceptance of the diet modification. • Determines dietary needs of patients from information gathered from patients chart in consultation with medical staff, and other sources. • Calculates special diets ordered by physicians. • Calculates weighted diets for patients whose intake must be specifically limited. • Observes, analyzes, and records diet program and patient response to diets to evaluate effectiveness of dietary program, to determine necessary diet changes or supplementary feeding, and to provide dietary information for related medical purposes. • Communicates directly with the patient’s PCP via the E.H.R. to ensure care coordination. • Counsels patients regarding normal and therapeutic dietary needs. • Confers regularly and periodically with the primary physician to review progress, exchange information and discuss future goals. • Assists in planning menus and nourishment lists as needed for conformity with dietary policies and standards based on the patient’s primary diagnosis (diabetes, overweight, hypertension, etc.). • Develops and conducts in-service training for personnel when appropriate. • May be required to submit written monthly reports • Practices confidentiality in regard to all client related matters. • Shall conduct group sessions for patients. • All other duties as assigned.

QUALIFICATIONS: Following are the qualifications required to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act. A combination of education, training and work experience equivalent to the following: • A Bachelor’s Degree in nutrition or dietetics. • Registered Dietitian Certification by the Commission of Dietetic Registration (the Credentialing Agency for the American Dietetic Association). Preferred Experience: • Experience working in a Federally Qualified Health Center (FQHC) • Familiarity with programs related to Community Health Centers/FQHC’s. Required Skills/Abilities: • Excellent people skills, with an ability to work effectively and collaboratively with a dynamic leadership & healthcare team. • Strong interpersonal and problem-solving skills. • Flexible, able to multitask; while setting priorities. • Computer knowledge, a must (Excel, Power Point, Word). • Strong communication skills, both verbal & written skills. Ability to communicate effectively. • Requires exceptional organizational skills. • Self-motivated to take initiative to ensure all tasks performed are completed and accurate. • Ability to multi-task, prioritize workload, and work under pressure of deadlines. • Ability to function self-sufficiently, requiring little supervision. • Commitment to the provision of high quality health care for under- served men, women, and children. Vision, hearing, manual dexterity and eye-hand coordination must be adequate for performance of job duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients, staff, visitors. Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.
