POSITION: Care Coordination Supervisor (Full Time Regular)

SHIFT: Monday – Friday @ 8am to 5pm                       

LOCATION: Corporate – Gardena, CA

*Some travel may be required to the Health Centers (Hawthorne, Wilmington, Carson, Santa Ana – CA)

Role and Responsibilities:

  • Direct and supervise all work activity of the Health Center Patient Care Coordinators.

  • Plan, organize, and coordinate the operation of referrals for specialty care coordination activities of the Health Center Network.

  • Provide oversight of daily operations for care coordination of medication assisted treatment (MAT), mental health, and other specialty services assigned to Patient Care Coordinators.

  • Develop, train, and implement care coordination policies.

  • Supervise, direct, train, and evaluate performance of Patient Care Coordinators.

  • Effective scheduling of staff and workflow to meet goals relating to utilization and quality.

  • Answer inquiries, provide information, and resolve complaints from providers, patients, or other agencies in regard to care coordination.

  • Assist with enrollment into applicable assistance programs (such as Medi-Cal or Covered CA), preregistering the patient with the health center, and connecting patients to resources. Follow-up on pending referrals.

  • Cooperatively work with outside agencies to help patients apply for alternate resources when appropriate.

  • Serve as the main point-of-contact for patient intake into case management programs offered by BHS Health Center Network.

  • Under the supervision of an RN or medical provider, devise procedures and protocols for patient intake, risk stratification, and filtration so that each enrolled patient receives consistent, attentive care.

  • Oversee intake, transfer of care and discharge activities for patients enrolled in specialty care coordination.

  • Provide care management of specialty referrals and linkages to and from community-based partners and other needed services related to a given program.

  • Communicate regularly with patients and their families about plans of care and treatment protocols.

  • Answer patients’ questions about their care, treatment plans, and all other issues so they feel safe and secure in our care.

  • Monitor and adjust patient statuses based on changing needs and conditions.

  • Coordinate referrals related to the enrolled program to and from BHS Health Center Network.

  • Coordinate case conferences with relevant care providers and resources.

  • Report all care management activities and outcomes to executive management.

  • Interview, hire, perform disciplinary action and performance evaluations for employees.

  • Maintain and foster relationships with community providers, agencies and organizations involved in care transitions and continuity of care for patients.

  • Works collaboratively as a team member with all staff members.

  • Promote and believe in BHS mission statement.

  • Other duties as assigned.

Required Knowledge, Skills and Abilities:

  • High School diploma, GED or equivalency.

  • Associate’s degree in healthcare or a related field.

  • Possession of current California Driver’s License.

  • Ability to relate to the public regardless of racial, ethnic, economic, sexual orientation, and gender identity status.

  • Experience working for a community clinic and/or with Medi-Cal and Medicare.

  • Knowledge of drug and alcohol dependency, mental health, family systems and/or 12-Step/recovery resources in the community.

  • Supervisory experience of at least two (2) years and knowledge of supervisory practices and principles, including work planning, scheduling, review and evaluation and employee training and discipline.

  • Ability to demonstrate/convey respect for cultural and lifestyle diversities of clients and staff.

  • Ability to present to diverse communities/populations, at all levels of internal/external organizations.

  • Ability to recognize personal issues that may have an impact on job performance and interactions with clients.

  • Ability to note documentation in electronic medical record system.

  • Strong verbal and written communication skills.

  • Proven track record of, and ability to demonstrate ethical and professional standards of behavior.

  • Basic knowledge of referrals, both in and out of the BHS treatment system.

  • Basic knowledge and Experience in utilizing Microsoft Office, specifically MS Word and Excel.

  • Ability to speak, understand, and read Spanish, especially Medical Spanish.

Desired Knowledge, Skills and Abilities:

  • Bachelor’s degree in healthcare or a related field.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear and use hands and fingers to operate a computer and telephone keyboard reach.

  • Light to moderate lifting is occasionally required (up to 25 lbs.)

  • Ability to sit at a computer terminal for an extended period of time.

  • Ability to travel locally to clinical sites including, but not limited to: Santa Ana, East LA, Wilmington, Carson, and Hawthorne.

Work Environment:

  • Moderate noise  (i.e. office with computers, printers, phone, light traffic)
  • Professional office environment.
