POSITION: Health Center Behavioral Health Navigator (Regular Full Time)

LOCATION: Family Health Center – Hawthorne, CA

DUTIES: Assists patients with Substance Use Disorder (SUD) and/or Mental Health (MH) in navigating the service delivery system including effectively overcoming barriers to care. Coordinates the care of clinic patients with specialty MH and SUD providers and assist with case management needs (adhere to care plan, medication compliance, etc.) Confronts client behavioral changes, identifies crises and recognizes when additional assistance is necessary. Builds collaborative support system with community employers, agencies and organizations, employing eligible clients as they move into transitional period. Occasionally provides transportation for clients. Other duties as assigned.

Required Knowledge: High school diploma or equivalency. Ability to build collaborative support system with community employers, agencies and organizations. Ability to demonstrate/convey respect for cultural and lifestyle diversities of clients and staff. Ability to present to diverse communities/populations, at all levels of internal/external organizations. Ability to recognize personal issues that may have an impact on job performance and interactions with clients. Ability to write documentation and progress reports.  Ability to note documentation in electronic medical record system. Strong verbal and written communication skills. Proven track record of, and ability to demonstrate ethical and professional standards of behavior. Basic knowledge of referrals, both in and out of the BHS treatment system. Basic knowledge and Experience in utilizing Microsoft Office, specifically MS Word/Excel/PowerPoint.

Desired Knowledge: Knowledge of drug and alcohol dependency, mental health, family systems and 12-Step/recovery resources in the community. Bachelor’s degree in Social Work or related field. AA degree or relevant Substance Abuse/Addiction Counselor education.

Possession of current California Driver’s License. While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear and use hands and fingers to operate a computer and telephone keyboard reach. Light to moderate lifting is occasionally required (up to 25 lbs.) Ability to sit at a computer terminal for an extended period of time. Ability to travel locally.

Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.
